Handling of Fragments during Endoscopic Combined Surgery

During endoscopic combined surgery, when you have fragments that you can only reach with the flexible ureteroscope but that can’t fit in the ureteral access sheath, you can leave the fragments in an appropriate place where the other surgeon can collect with the nephroscope. Additionally, you can pass them out via the Amplatz sheath simply by placing the tip of the flexible ureteroscope on the opening of the Amplatz sheath and pushing the basket further out of the ureteroscope until the fragment that is grasped by the basket comes out of the Amplatz sheath from the percutaneous access side of the patient.
